Required Documents

Proof of Name Change

The document must be a court-ordered document.

Proof of Identification

Proof of Social Security

  • Social Security Card that is signed (If the driver is changing their name on their CDL license, the original card is the only acceptable proof of Social Security)
  • W-2 form
  • Pay Stub showing applicant’s name and Social Security number

Proof of Residency

  • Mortgage papers or rental contract
  • Major credit card bill (dated no later than sixty days)
  • Utility bill (dated no later than sixty days)
  • Valid Utah registration or title
